在預約之前請注意以下事項

如果您是第一次來診，請您填寫並攜帶患者登記表格以及症狀問卷。

您甚至可以提前通過電子郵件或傳真傳送給我們填寫完整的表格，以避免延誤您的預約時間。

請提前10分鐘到達，以便有時間完成註冊過程。

請帶上以下內容：

  • 患者登記表格以及症狀問卷 – Patient Registration Form, and if applicable, Foot and Ankle Questionnaire
  • 轉診信 – Referral Letter
  • 任何影像學研究（X射線，超聲波，CT掃描，MRI掃描，核醫學研究）和報吿 – Any imaging studies (X-rays, ultrasounds, CT scans, MRI scans, nuclear medicine studies) and reports if available
  • 任何其他相關調查（神經傳導研究等）- Any other relevant investigations (nerve conduction studies etc)
  • 任何操作報告或解除摘要 – Any operation reports or discharge summaries
  • Medicare卡和適用的私人醫療保險會員卡，工傷賠償索賠號，第三方索賠資料 – Medicare card and if applicable, private health insurance membership card, Workers Compensation claim number, 3rd Party claim details
